Online Medical Registry participation is free with the purchase of a Universal Medical ID. Your Online Medical Registry will be added to your cart automatically with any ID purchase. For those who already own a medical ID or are unable to wear one, an Online Medical Registry may be purchased for a $32.95 one-time charge. You'll then be a permanent member with no annual or renewal fees. Following enrollment you'll be given instructions on how to set up your Online Medical Registry file.

All previous Universal Medical ID customers are eligible to receive a Free Online Medical Registry. During registration you will need to provide your previous Universal Medical ID order number, if available, and your Online Medical Registry access code. If you need assistance please contact customer service.
